Registration forms for congregational delegates and for individuals are attached as PDF documents. They will need to be completed and mailed with a check with the registration fee. Please note that delegates from a congregation should not send in individual registration forms; they should be registered together with the other delegates from their congregation on the Congregational Registration Form. Only nonparochial clergy and lay people who wish to attend as visitors should use the Individual Registration Form.
